Immunization Requirements
Texas law requires proof of immunization against Diphtheria, Tetanus, Pertussis, Poliomyelitis, Hepatitis B, and HIB, and proof of
immunization or a history of illness from Rubella, Rubeola, Mumps and Varicella. Pre-k students are required to have Hepatitis A and
Pneumococcal. This is required of every student, according to prescribed school grade criteria, seeking admission to or enrolled in
any school in the Overton Independent School District.
Diphtheria/Pertussis/Tetanus/Td
(DTP, DTaP, DT, Td)
• Five doses of any combination DTaP/DTP unless 4th dose was received on or after the 4th
birthday in which case only 4 doses are required. Students who started their vaccinations after
age 7 are required to have at least 3 doses of a tetanus-diphtheria containing vaccine. Receipt of
the dose up to and including 4 days before the 4th birthday will satisfy the school entry
immunization requirement. One dose of Td required ten years after last dose of DTP/DTaP/DT.
Polio
• Four doses are required, one of which must be on or after the fourth birthday. If the 3rd dose
was given on or after the fourth birthday, only 3 doses are required. If any combination of four
doses of OPV and IPV was received before four years of age, no additional dose is required.
Receipt of the dose up to and including 4 days before the 4th birthday will satisfy the school
entry immunization requirement. If 18 years or older, not required.
Measles, Mumps, Rubella
(MMR)
• Two doses of a measles-containing vaccine with the 1st dose on or after the 1st birthday; 2nd dose
by age 5 or entry into kindergarten. One dose of rubella vaccine and one dose of mumps vaccine
received on or after the 1st birthday are required. Receipt of the dose up to and including 4 days
before the 1st birthday will satisfy the school entry immunization requirement.
• Serologic confirmation of immunity to measles, mumps, rubella, or serologic evidence of
infection is acceptable in lieu of vaccine.
Hepatitis B
Hepatitis A
• Three doses are required for the following grades in the following school years:
2004-05, K-5 and 7-10
2005-06, K-11
2006-07, K-12
• Serologic confirmation of immunity to hepatitis B or serologic evidence of infection is
acceptable in lieu of vaccine.
• Two doses of adult hepatitis B vaccine (Recombivax) are acceptable. Dosage and type of
vaccine must be clearly documented (Two 10mcg/1.0ml of Recombivax).
• Pre-k students, two doses given at least 6 months apart and no longer than 18 months from the
date of the first dose.
Varicella (Chickenpox)
Pneumococcal
• One dose on or after the 1st birthday for the following grades in the following school years:
2004-05, K-4 and 7-10
2005-06, K-5 and 7-11
2006-07, K-12
Two doses if vaccine given at 13 years of age or older.
• Serologic confirmation of immunity to varicella or documentation of previous illness may
substitute for vaccination.
• For Pre-k students, a series of three shots given prior to age 1 with a booster after 1 year. If no
prior doses given, one dose required. Not given after age 5.
HIB
• Required for children age 15 months through 4 years. One dose on or after age 15 months,
unless scheduled for a primary series (2 doses before the 1st birthday and a booster after the 1st
birthday).
Medical Exemption: The law allows physicians to write a statement stating that the vaccine(s) required would be medically harmful
or injurious to the health and well being of the child. The written statement by the physician must be submitted to the school. Unless it
is written in the statement that a lifelong condition exists, the exemption statement is valid for only one year from the date signed by
the physician.
Conscientious/Religious Exemption: The law allows parents/guardians to choose an exemption from immunization requirements
for reasons of conscience, including a religious belief. Instructions for the affidavit to be signed by parents/guardians can be found at
www.ImmunizeTexas.com. The completed and returned form from the TDH must be submitted to the school. The affidavit will be
valid for two years.
Please note: In the case of an outbreak, an un-immunized child may be prohibited from school attendance.
